nurses, and entire staff are committed to providing you with excellent care
as our patient. In addition, it is our policy to respect your privacy and
dignity. Our sincere intention is to deliver your medical care professionally
and compassionately. We encourage you to read the following pages for a
full understanding of your rights and responsibilities as a patient.
Every patient of Commonwealth Endoscopy Center shall have the right:
1. To freedom of his or her selection of a physician
except in the case of an emergency;
2. Upon request, to obtain the name and specialty of
the physician or other person responsible for his/her care;
3. To confidentiality of all records and communication
to the extent provided by law;
4. To have all reasonable requests responded to promptly
and adequately within the capabilities of this center;
5. To privacy during treatment or care;
6. To informed consent to the extent provided for by
law; and
7. Upon request, to inspect his or her medical records
and to receive a copy for a fee determined by the current rate of copying
expenses;
8. Upon request, to receive an itemized bill of charges
to any third party by the facility and to receive any information this
center has available relative to financial assistance and free care;
9. Upon request, to obtain an explanation as to the
relationship, if any, of Commonwealth Endoscopy Center to any other health
care facility or educational institution in so far as it relates to his
care;
10. To refuse to be examined, observed or treated by
students or any other center staff without jeopardizing access to psychiatric,
psychological or other medical care of attention;
11. To refuse to serve as a research subject and to
refuse any care or examination when the primary purpose is educational
or informational ratherthan therapeutic;
12. To prompt life saving treatment in an emergency
without discrimination because of source of payment unlesssuch delay will
not be a risk to thepatient’s health;
13. To obtain a copy of any rules or regulations of
this facility which apply to his or her conduct as a patient.
